Family Practice Physician
Family Healthcare of Hagerstown is seeking a full-time physician to provide primary office care in an outpatient setting. This staff must have completed education at an accredited medical school and residency program. She/he must be Board Certified or eligible. The following licensures are required: physician licensure in the State of Maryland; CDS license in the State of Maryland; and DEA license. This stuff must hold current BLS or ACLS certification. Hours will be 8:00 a.m. to 5:00 p.m. daily Monday through Friday (no weekends and no major holidays) and occasional Tuesday evenings. Responsibilities include working with other providers during the same hours in a collegial atmosphere, addressing acute or same-day medical care issues with some routine medical care for primary/preventive care and management, of chronic conditions. The ideal candidate will have at least one year of experience in a medical office setting, though other clinical areas may be substituted. Additionally, he or she must be licensed or eligible for licensure in the State of Maryland as a Nurse Practitioner, as a Controlled Dangerous Substances Provider, and by the US Drug Enforcement Administration (DEA). Family Healthcare of Hagerstown is seeking an experienced, contractual, Maryland licensed Psychiatrist to perform monthly peer reviews of patient electronic health records. Salary and hours are negotiable.
LCSW-C or LCPC (Experienced in Working with Children and Adults)
Because Family Healthcare of Hagerstown is implementing an integrated pediatric mental health program, the Center is seeking LCSW-C or LCPC therapists who are experienced in providing care to both adults and children. Under the clinical direction and supervision of the attending physicians and psychiatrist, this employee independently assesses, evaluates, treats, educates, and monitors individuals who are patients of Family Healthcare of Hagerstown and who are experiencing emotional and/or mental disorders as defined by the latest version of the DSM. The person shall perform job duties that are consistent with a model that integrates primary mental health care with primary medical care. The candidate will demonstrate a high level of skill necessary to perform the job duties within regulatory requirements. He/Sher will have extensive experience working with diverse populations in an integrated medical facility and will be a crucial member of our team. Call Center Operator
This full-time position provides clerical, secretarial, and patient scheduling activities to ensure the prompt and efficient care of the patients to meet the goals and objectives of Family Healthcare of Hagerstown (FHH). He/She is responsible for appropriately addressing incoming telephone calls, scheduling patients, verifying insurance coverage, taking messages, completing reports, faxing, copying, and utilizing a computer. The person will maintain respect for employees and volunteers, be committed to the mission, and provide quality service. Graduation from high school with commercial/business courses or the equivalent is required. Graduation from Medical/Secretarial program is preferred. The candidate will have experience in working with computers and a knowledge of medical terminology. Experience with an electronic medical record (EMR) is preferred. He/She must have excellent communication skills, both written and oral. The position includes minimal physical effort with some walking, bending, lifting, reaching, etc. Frequent periods of concentration, attention to detail, and multi-tasking are required.
Community Outreach Worker
This is part-time (20 hours/week) contractual position; therefore, benefits are not provided. This position provides assistance to Family Healthcare of Hagerstown patients. Activities include helping community residents and patients to access services such as transportation, primary care, dental care, behavioral health services, substance abuse, labs, x-rays and specialty care services. The employee will develop and maintain ongoing community involvement with appropriate agencies and organizations. Graduation from high school with commercial/business courses or the equivalent is required, and graduation from a Medical/Secretarial program is preferred. Experience must include computer skills and, EMR and Medical terminology experience is preferred. The staff will exhibit excellent communication skills as well as the ability to speak, write and understand the English language. The ability to work independently is important. The ideal candidate will have direct knowledge of local community resources and programs.
This full-time staff assists with greeting patients, scheduling appointments, answering phones, and other office responsibilities. Under the direction of the Dental Supervisor, this position ensures patients have a current treatment plan and that patients follow through with it. The position provides coordination of financial resources and/or insurance requirements. These objectives are achieved by understanding dental benefits and discussing them and associated financial responsibilities with patients. The staff will submit pre-authorizations, maintain call lists, track patients that are placed on a dental grant, and run closing reports to ensure funds are balanced. The employee must exhibit regard and respect for all patients, visitors, and staff. He/She will support and follow through with the vision, mission, goals and objectives of FHH. Graduation from high school with commercial/business courses or the equivalent is required; graduation from a Medical/Secretarial/Dental program is preferred. Computer skills and knowledge of dental terminology are required.
This full-time employee will perform general housekeeping tasks in order to maintain a clean, safe, sanitary, orderly, attractive, and healthy environment for patients, visitors, and staff. He/She will ensure that all patient areas are maintained daily in accordance with protocols. This includes, housekeeping of exam rooms and operatories, bathrooms, floors, lounge, trash, outdoors, etc. Appropriate supplies must be utilized and maintained. The person will follow policies related to infection control and standard precautions. He/She will demonstrates regard for, dignity toward, and respect for all patients and visitors. A high school diploma or equivalent is preferred and the candidate will have basic skills in environmental cleanliness and infection control. Six months previous commercial building cleaning experience preferred. The person must be able to perform heavy duty cleaning which includes lifting up to 40 pounds, working from step ladders, dexterity to operate cleaning equipment, etc. The position entails period of concentration and attention to detail. This position is directly supervised by the Operations Manager.
LPN Pod Coordinator
This full-time position works within a clinical team setting to provide general and specialized nursing care to ensure patient needs are met in a professional and timely manner. Responsibilities include coordination of patient care on the pod, the processing of medication refills, the review and report of lab results providers and patients as directed. He/She functions as a resource to teammates and assists with training of new clinical staff. Requirements: current MD state LPN license, current CPR certification, basic computer skills, excellent interpersonal skills. EMR experience and Family Practice experience is preferred. Hours: full-time.
LPN Pre-visit Coordinator
Under the direction of the Clinical Supervisor, the full-time Pre-visit Coordinator/LPN will provide chronically ill patients and recently hospitalized patients telephone and face-to-face support before the first patient to provider visit so care can be addressed, lengthy medication reconciliations can be completed, and complex social/medical histories ascertained and organized. The staff will provide general and specialized nursing care to ensure patient needs are met in a professional and timely manner. The candidate will support and uphold the vision, mission, goals, and objectives of FHH. Part of the skill set must include providing good customer service, documenting appropriately in the EHR, utilizing computers and standard office equipment, etc. The candidate must hold a current LPN license to practice in the State of Maryland as well as CPR Certification. Clinical Family Practice experience preferred.
This full-time position provides general and specialized nursing care to ensure needs are met in a professional and timely manner. Licensures required: must have received a certificate or training indicating satisfactory completion of a Medical Assistant program; current CPR Certification. Experience: Family Practice preferred; typing skills to enter data in EHR efficiently. Hours: Monday through Friday – varied hours.
RN Case Manager
This full-time employee is responsible for managing chronic illness patients in a patient-centered model to promote effective education, self-management support, and timely healthcare delivery to achieve optimal quality outcomes. The employee will collaborate with the care team and coordinate treatment while prioritizing patients according to complexity and needed follow-up. The person will maintain databases, provide timely documentation, and schedule patients appropriately. He/She will identify and utilize community resources as necessary and according to plan. The role includes coverage of the triage phone, gathering statistics for grants, and maintenance of AED’s. The candidate must have a current Maryland RN license; prior family practice case management experience is preferred.
