Family Practice Physician
Family Healthcare of Hagerstown is seeking a full-time physician to provide primary office care in an outpatient setting. This staff must have completed education at an accredited medical school and residency program. She/he must be Board Certified or eligible. The following licensures are required: physician licensure in the State of Maryland; CDS license in the State of Maryland; and DEA license. This staff must hold BLS or ACLS certification. Hours will be 8:00 a.m. to 5:00 p.m. daily Monday through Friday (no weekends and no major holidays) and occasional Tuesday evenings. Responsibilities include working with other providers during the same hours in a collegial atmosphere, addressing acute or same-day medical care issues with some routine medical care for primary/preventive care and management of chronic conditions. This staff must be proficient in utilizing EHR systems. This position comes with a competitive salary and an excellent benefits package.
LCSW-C or LCPC (Experienced in Working with Children and Adults)
Because Family Healthcare of Hagerstown is implementing an integrated pediatric mental health program, the Center is seeking LCSW-C or LCPC therapists who are experienced in providing care to both adults and children. Under the clinical direction and supervision of the attending physicians and psychiatrist, this employee independently assesses, evaluates, treats, educates, and monitors individuals who are patients of Family Healthcare of Hagerstown and who are experiencing emotional and/or mental disorders as defined by the latest version of the DSM. The person shall perform job duties that are consistent with a model that integrates primary mental health care with primary medical care. The candidate will demonstrate a high level of skill necessary to perform the job duties within regulatory requirements. He/Sher will have extensive experience working with diverse populations in an integrated medical facility and will be a crucial member of our team. This candidate must be goal driven, highly motivated, detail-oriented, creative in delivery of services, and devoted to providing excellent patient care. This position requires licensure in the State of Maryland. This position includes a sign-on bonus of $5,000.
Nurse Practitioner
Family Healthcare of Hagerstown is seeking a Certified Nurse Practitioner to provide comprehensive care to patients. The ideal candidate will have at least one year of experience in a medical office setting, though other clinical areas may be substituted. Additionally, he or she must be licensed or eligible for licensure in the State of Maryland as a Nurse Practitioner, as a Controlled Dangerous Substances Provider, and by the US Drug Enforcement Administration (DEA). Responsibilities include medical diagnosis and treatment; the completion of appropriate documentation in the Electronic Health Record; participation in peer meetings; participation in, and support of, patient satisfaction activities; participation in, and support of, quality assurance activities; adherence to strict confidentiality requirements; positive communication amongst staff and patients; and participation in professional development opportunities.
Pediatric Dentist
This part time (3-4 days per week) position will provide dental care to pediatric patients through evaluation, diagnosis, treatment, and restoration at the FHH Dental Practice to include the Healthy Smiles in Motion, mobile dental unit. This staff works under the direction of the Dental Director. Comprehensive dental diagnosis and treatment includes but is not limited to providing evaluations using history, oral examination, and radiographs; determining plans for treatment; providing treatment and restoration; counseling patients and/or families of oral health matters; referring patients for specialty programs; responding to urgent dental situations; and actively participating in an on-call rotation. The provider will also participate in quality improvement efforts such as peer reporting. Computer proficiency to include experience with electronic patient records is required. The candidate must have graduated from an accredited dental school, hold a current license to practice in the state of Maryland, have a DEA license as well as a Maryland State Dispensing License, and have current BLS and/or ACLS certification. Physician Assistant
The organization seeks a Physician Assistant to provide comprehensive care to patients in a Family Practice setting. The position entails all aspects of care including evaluation, diagnosis, and treatment. The candidate must have a BS from an AMA accredited Physician Assistant Program. He or she must be certified by the National Commission on Certification of Physician Assistants and a graduate of a Physician Assistant program accredited by the Review Commission on Education for the Physician Assistant. The ideal candidate will have at least one year of experience in a medical office setting, though other clinical areas may be substituted. Additionally, the candidate must be licensed or eligible for licensure as a Physician’s Assistant in the State of Maryland; hold a Maryland Controlled Dangerous Substance license; hold a DEA License; and be BLS or ACLS certified.
Psychiatric Consultant

Community Outreach Worker
This full-time position provides assistance to Family Healthcare of Hagerstown patients. Activities include helping community residents and patients to access services such as transportation, primary care, dental care, behavioral health services, substance abuse, labs, x-rays and specialty care services. The employee will develop and maintain ongoing community involvement with appropriate agencies and organizations. Graduation from high school with commercial/business courses or the equivalent is required, and graduation from a Medical/Secretarial program is preferred. Experience must include computer skills and, EMR and Medical terminology experience is preferred. The staff will exhibit excellent communication skills as well as the ability to speak, write and understand the English language. The ability to work independently is important. The ideal candidate will have direct knowledge of local community resources and programs.
Dental Assistant
Under the direct supervision by a licensed dentist as permitted by the Maryland COMAR regulations, this full-time employee performs general chair side and clinical functions related to patient dental care in the office, in the mobile dental unit, and possibly in the operating room at Meritus Medical Center. The person shares responsibility with the dental team; he/she operates x-ray equipment, develops templates, and prepares operatories. This person also prepares the patient for treatment and helps to explain procedures. A high school diploma and successful completion of a nationally accredited dental assistant program is a must. He or she will maintain certification to ensure privileging at Meritus Medical Center to assist in dental procedures if operating room care is provided by the employee; maintain DANB by continuing education and community resources; maintain CPR certification, etc. A minimum of 1-2 years of experience as a dental assistant with pediatric patients is preferred. Like other staff, this person supports and follows through with vision, mission, goals, and objectives of the Family Healthcare of Hagerstown.

LPN
This is a full time position that provides general and specialized nursing care to ensure needs are met in a professional and timely manner. Licensures required include a current LPN license to practice in the State of Maryland and current CPR Certification. Family Practice experience is preferred. The employee should have typing skills to efficiently and accurately enter data in the electronic health record. Hours are varied through the business week of Monday through Friday.
Medical Assistant
This full-time position provides general and specialized nursing care to ensure needs are met in a professional and timely manner. The candidate must have have received a certificate or training with proof that indicates satisfactory completion of a Medical Assistant program. Additionally, the candidate must be currently CPR certified. She/He will exhibit regard and respect for all patients, visitors, and staff while following through on the vision, mission, goals and objectives of FHH. Experience: pediatric experience preferred; typing skills to enter data in EHR efficiently. Hours: Monday through Friday – varied hours.
Referral Specialist
This full-time staff serves as the primary resource for arranging and scheduling authorized patient referrals to specialists, for diagnostic procedures, and surgery. He/She will obtain necessary pre-authorization approval and will refer patients to authorized providers as required by the insurance and/or managed care organization to ensure that patients’ medical needs are met in a timely and professional manner. The referral specialist must have a working knowledge of clerical functions such as faxing, taking messages, copying records, computer usage, etc. A knowledge of medical terminology is also required. EMR experience or the ability to learn it is important. Previous referral, insurance verification, billing, and/or medical office experience is preferred. Graduation from high school with commercial/business courses or the equivalent is required; graduation from a Medical/Secretarial program is preferred. This staff must have excellent communication skills as well as the ability to speak, write and understand the English language.
RN Case Manager
This full-time employee is responsible for managing chronic illness patients in a patient-centered model to promote effective education, self-management support, and timely healthcare delivery to achieve optimal quality outcomes. The employee will collaborate with the care team and coordinate treatment while prioritizing patients according to complexity and needed follow-up. The person will maintain databases, provide timely documentation, and schedule patients appropriately. He/She will identify and utilize community resources as necessary and according to plan. The role includes coverage of the triage phone, gathering statistics for grants, and maintenance of AED’s. The candidate must have a current Maryland RN license; prior family practice case management experience is preferred.
