The Center is seeking both general and/or pediatric dentists in a full-time and/or part-time capacity. Under the direction of the Dental Director, the candidate will practice dentistry in the stationary office or on a Healthy Smiles in Motion mobile dental unit; both settings provide full-scale, comprehensive dentistry. This position will provide dental evaluation, diagnosis, treatment, and restoration. Additional responsibilities include the completion of appropriate charting and coding of services in the electronic medical record; appropriate response to dental emergencies; and participation in Quality Improvement measures such as monthly peer audits. He/She will support patient satisfaction efforts and exercise confidentiality. Courteous communication skills with patients, visitors, and staff must be demonstrated. The position supports and follows through with the vision, mission, goals, and objectives of the Family Healthcare of Hagerstown. Qualifications include completion of an accredited dental school, current Maryland licensure, DEA and Maryland State Dispensing License, and current BLS and/or ACLS. Two years of experience in dentistry is preferred. (Note: FHH is an eligible NHSC loan repayment site with a Dental HPSA score of 21.)
Full-time physician to do primary office care in an outpatient setting. Must have competed accredited medical school and residency program. Board Certified or eligible. Licensures required: Physician licensure in the State of Maryland; CDS license in the State of Maryland; DEA license. Current BLS or ACLS certified. Hours: 8:00 a.m. to 5:00 p.m. daily Monday through Friday (no weekends and no major holidays); occasional Tuesday evenings. Responsibilities: Work with other providers during the same hours in a collegial atmosphere; emphasis on acute or same-day medical care issues with some routine medical care for primary/preventive care and management of uncomplicated chronic conditions. Experience: Seeking provider with at least two years work experience in a family practice setting preferred (including residency); must be proficient in utilizing EHR system. Family Healthcare of Hagerstown is seeking a Certified Nurse Practitioner to provide comprehensive care to patients. The ideal candidate will have at least one year of experience in a medical office setting, though other clinical areas may be substituted. Additionally, he or she must be licensed or eligible for licensure in the State of Maryland as a Nurse Practitioner, as a Controlled Dangerous Substances Provider, and by the US Drug Enforcement Administration (DEA). Responsibilities include medical diagnosis and treatment; the completion of appropriate documentation in the Electronic Health Record; participation in peer meetings; participation in, and support of, patient satisfaction activities; participation in, and support of, quality assurance activities; adherence to strict confidentiality requirements; positive communication amongst staff and patients; and participation in professional development opportunities.

Family Healthcare of Hagerstown is seeking an experienced, contractual, Maryland licensed Psychiatrist to perform monthly peer reviews of patient electronic health records. Salary and hours are negotiable.
Under the clinical direction and supervision of the attending physicians and psychiatrist, this employee independently assesses, evaluates, treats, educates, and monitors individuals who are patients of Family Healthcare of Hagerstown and who are experiencing emotional and/or mental disorders as defined by the latest version of the DSM. The person shall perform job duties that are consistent with a model that integrates primary mental health care with primary medical care. The candidate will demonstrate a high level of skill necessary to perform the job duties within regulatory requirements. He/Sher will have extensive experience working with diverse populations in an integrated medical facility and will be a crucial member of our team. This candidate must be goal driven, highly motivated, detail-oriented, creative in delivery of services, and devoted to providing excellent patient care. This position requires licensure in the State of Maryland. This position includes a sign-on bonus of $5,000.

This staff assists with greeting patients, scheduling appointments, answering phones, and other office responsibilities. Under the direction of the Dental Supervisor, this position ensures patients have a current treatment plan and that patients follow through with it. The position provides coordination of financial resources and/or insurance requirements. These objectives are achieved by understanding dental benefits and discussing them and associated financial responsibilities with patients. The staff will submit pre-authorizations, maintain call lists, track patients that are placed on a dental grant, and run closing reports to ensure funds are balanced. The employee must exhibit regard and respect for all patients, visitors, and staff. He/She will support and follow through with the vision, mission, goals and objectives of FHH. Graduation from high school with commercial/business courses or the equivalent is required; graduation from a Medical/Secretarial/Dental program is preferred. Computer skills and knowledge of dental terminology are required.
RN Case Manager
This employee is responsible for managing chronic illness patients in a patient-centered model to promote effective education, self-management support, and timely healthcare delivery to achieve optimal quality outcomes. The employee will collaborate with the care team and coordinate treatment while prioritizing patients according to complexity and needed follow-up. The person will maintain databases, provide timely documentation, and schedule patients appropriately. He/She will identify and utilize community resources as necessary and according to plan. The role includes coverage of the triage phone, gathering statistics for grants, and maintenance of AED’s. The candidate must have a current Maryland RN license; prior family practice case management experience is preferred.
