Clinical Services Director
This full-time position provides leadership and administrative direction for clinical support staff to include primary care and integrated mental health staff. In addition, she/he will provide direct patient care.
The staff person will support Strategic Plan and budgetary goals by assisting with leadership activities related to accreditations, certification, or compliance: FTCA, Quality Management, Risk Management, Quality Assurance, HIPAA regulations, patient confidentiality policies, and medically-related policies and procedures. The staff will work with RN Clinical Supervisor to determine clinical support staffing needs and lead in recruitment and hiring efforts. She/He will provide direct supervision to theRN Clinical Supervisor and indirect supervision of Clinical Support Staff. This includes assisting in the review of clinical quality measures, ensuring clinical staff are providing quality patient care, and that they are documenting such. This Director will participate in evaluations and oversee scheduling. As a care provider, the candidate will evaluate, examine, and counsel patients medically as well as order tests, referrals, prescriptions, and provide indicated procedures. The candidate must have completed an accredited medical school and residency program. Additionally, an MPH, Master’s Degree in Healthcare Administration, or similar advanced degree is preferred. He/She will be Board Certified within two years of the fellowship program and hold a Maryland medical license, DEA license, Maryland State Dispensing License, and current BLS and/or ACLS certification. Prior experience as a clinical director in an FQHC, group practice or hospital preferred. This position works under the direction of the Executive Director and the decision- making role of the Board of Directors.
The Center is seeking a general dentist in a full-time and/or part-time capacity. Under the direction of the Dental Director, this position will provide dental evaluation, diagnosis, treatment, and restoration. Additional responsibilities include the completion of appropriate charting and coding of services in the electronic medical record; appropriate response to dental emergencies; and participation in Quality Improvement measures such as monthly peer audits. He/She will support patient satisfaction efforts and exercise confidentiality. Courteous communication skills with patients, visitors, and staff must be demonstrated. The position supports and follows through with the vision, mission, goals, and objectives of the Family Healthcare of Hagerstown. Qualifications include completion of an accredited dental school, current Maryland licensure, DEA and Maryland State Dispensing License, and current BLS and/or ACLS. Two years of experience in dentistry is preferred. (Note: FHH is an eligible NHSC loan repayment site with a Dental HPSA score of 25.)
Family Practice Physician
Family Healthcare of Hagerstown is seeking a full-time physician to provide primary office care in an outpatient setting. This staff must have completed education at an accredited medical school and residency program. She/he must be Board Certified or eligible. The following licensures are required: physician licensure in the State of Maryland; CDS license in the State of Maryland; and DEA license. This staff must hold BLS or ACLS certification. Hours will be 8:00 a.m. to 5:00 p.m. daily Monday through Friday (no weekends and no major holidays) and occasional Tuesday evenings. Responsibilities include working with other providers during the same hours in a collegial atmosphere, addressing acute or same-day medical care issues with some routine medical care for primary/preventive care and management of chronic conditions. The ideal candidate will have at least one year of experience in a medical office setting, though other clinical areas may be substituted. Additionally, he or she must be licensed or eligible for licensure in the State of Maryland as a Nurse Practitioner, as a Controlled Dangerous Substances Provider, and by the US Drug Enforcement Administration (DEA). Family Healthcare of Hagerstown is seeking an experienced, contractual, Maryland licensed Psychiatrist to perform monthly peer reviews of patient electronic health records. Salary and hours are negotiable.
LCSW-C or LCPC (Experienced in Working with Children and Adults)
Because Family Healthcare of Hagerstown is implementing an integrated pediatric mental health program, the Center is seeking LCSW-C or LCPC therapists who are experienced in providing care to both adults and children. Under the clinical direction and supervision of the attending physicians and psychiatrist, this employee independently assesses, evaluates, treats, educates, and monitors individuals who are patients of Family Healthcare of Hagerstown and who are experiencing emotional and/or mental disorders as defined by the latest version of the DSM. The person shall perform job duties that are consistent with a model that integrates primary mental health care with primary medical care. The candidate will demonstrate a high level of skill necessary to perform the job duties within regulatory requirements. He/Sher will have extensive experience working with diverse populations in an integrated medical facility and will be a crucial member of our team. This candidate must be goal driven, highly motivated, detail-oriented, creative in delivery of services, and devoted to providing excellent patient care. This position requires licensure in the State of Maryland. This position includes a sign-on bonus of $5,000.

Community Outreach Worker
This is part-time (20 hours/week) contractual position; therefore, benefits are not provided. This position provides assistance to Family Healthcare of Hagerstown patients. Activities include helping community residents and patients to access services such as transportation, primary care, dental care, behavioral health services, substance abuse, labs, x-rays and specialty care services. The employee will develop and maintain ongoing community involvement with appropriate agencies and organizations. Graduation from high school with commercial/business courses or the equivalent is required, and graduation from a Medical/Secretarial program is preferred. Experience must include computer skills and, EMR and Medical terminology experience is preferred. The staff will exhibit excellent communication skills as well as the ability to speak, write and understand the English language. The ability to work independently is important. The ideal candidate will have direct knowledge of local community resources and programs.
This full-time staff assists with greeting patients, scheduling appointments, answering phones, and other office responsibilities. Under the direction of the Dental Supervisor, this position ensures patients have a current treatment plan and that patients follow through with it. The position provides coordination of financial resources and/or insurance requirements. These objectives are achieved by understanding dental benefits and discussing them and associated financial responsibilities with patients. The staff will submit pre-authorizations, maintain call lists, track patients that are placed on a dental grant, and run closing reports to ensure funds are balanced. This is a temporary position (less than 1 year) with benefits. Under the direction of the Patient Access Manager, greets patients and guests entering the building. He or she will screen patients and visitors prior to entering the building by taking temperatures and asking COVID-19 screening questions. This person will collect and sign for incoming/outgoing packages, direct patients to the registrar, and assist with other tasks as assigned. Qualifications include graduation from a high school and commercial/business courses or the equivalent is required. The candidate must be able to work independently, have excellent communication skills and have a good mastery of the English language.
This full-time position provides general and specialized nursing care to ensure needs are met in a professional and timely manner. The candidate must have have received a certificate or training with proof that indicates satisfactory completion of a Medical Assistant program. Additionally, the candidate must be currently CPR certified. Under the direction of the Patient Access Manager, the staff is responsible for processing all requests for medical/dental information and records manually as well as from computerized system. He/She processes and files all incoming reports and records. The person will demonstrate a knowledge of the clerical functions such as faxing, taking messages, copying records, computer usage and scanning. Tasks include maintaining a log of patient who leave the practice and documenting the departure in the patient chart; gathering and completing medical record requests; scanning and filing documents electronically; responding to external medical records requests appropriately; receiving and distributing faxes; counting money, etc. The person must be a high school graduate with commercial/business courses, and graduation from a medical/secretarial program is preferred. Computer skills and a knowledge of medical terminology are required. EMR experience is preferred. Excellent communication skills and independence are a must.
RN Case Manager
This full-time employee is responsible for managing chronic illness patients in a patient-centered model to promote effective education, self-management support, and timely healthcare delivery to achieve optimal quality outcomes. The employee will collaborate with the care team and coordinate treatment while prioritizing patients according to complexity and needed follow-up. The person will maintain databases, provide timely documentation, and schedule patients appropriately. He/She will identify and utilize community resources as necessary and according to plan. The role includes coverage of the triage phone, gathering statistics for grants, and maintenance of AED’s. The candidate must have a current Maryland RN license; prior family practice case management experience is preferred.
